The receptionists are available between these times.

Every alternative week we are offering later routine appointments on a Monday & Tuesday from 6-6.30pm where a public holiday falls on the alternative week appointments will be offered on the Tuesday of that week please ask at reception for further details.

Telephone Consultations

If you require to speak to the Doctor on the telephone the best time to phone is between 11:30 and 12:00 am & 2pm and 3pm.

New Patients

If you wish to register with the practice, please contact the reception for a registration form and new patient health check questionnaire. You are then invited to make an appointment with the nurse for a New Patient Medical. On the completion of registration form a new medical card will be sent out to you.

Making an Appointment

To make an appointment with your GP please telephone the surgery on (01821) 642248. Every effort will be made to accommodate your needs for preferred time and GP.

If you are unable to keep an appointment please phone and cancel as soon as possible so that your appointment can be offered to someone else.

Home Visits

Contact the surgery on (01821) 642248 by 10am if possible with details of patient’s name, date of birth and details of symptoms.

Out of Hours

If you need to speak to or see a doctor in an emergency after 6pm when the surgery is closed, you can telephone the surgery as normal and you will hear a recorded message, this message will give you the new NHS24 out of hours number to contact.

To contact NHS24 the number is 08454 24 24 24, but please note this service is for EMERGENCIES ONLY. Any difficulty contacting NHS 24 dial operator on 100

Practice Nurse

Wendy Penny RGN is the senior practice nurse. Appointments can be made with Wendy for: Well Woman and Man Diabetes Review, Ear Syringe, Travel Vaccinations, Smoking cessation, ECG and other nursing duties.

Lynn Thomson is the practice Health Care Assistant. Appointments can be made with Lynn everyday for Blood Tests, Blood Pressure, New Patient Medical checks and ECG.

Practice Staff

The practice manager is Mrs. Susan Campbell. She is responsible for the administration of the practice and the staff. Mrs. Campbell is available to answer any questions or concerns you may have regarding the practice. If Susan is unavailable Mrs. Cilla Coupar assistant PM will be glad to help. Our reception staff are: Mrs. Sylvia Barnett & Mrs. Karen Dixon

The Health Visitor

The Health Visitor can be contacted on (01821) 641167. If you wish to make an appointment with the Health Visitor please contact reception on (01821) 642248. Her role within the practice is to administer childhood immunisations, baby clinics, weight checks and child screening.

District Nurse

The District Nursing Team can be contacted on (01821) 642238. If there is no answer please leave message on machine.

Medical Students

Medical students are sometimes present within the practice and we are grateful to our patients for their continued co-operation with this.

Services Available

The Practice provides a full range of general medical services and also provides the following through appointment only:

  • ·                Childhood Immunisation and vaccination
  • ·                Cervical Screening
  • ·                Immunisation and vaccination (flu vaccinations)

 

Results

Please note it is the patient’s responsibility to contact the surgery for their results. Please contact reception after 2pm. Please note that we require patient consent for results to be given out to anyone other than themselves.

Repeat Prescriptions

Repeat Prescriptions can be obtained by phoning the hotline number on (01821) 642002 or by post/in person. Unless otherwise stated your prescription will be passed to the Carse Chemist for dispensing within 48hrs. Arrangements can be made for your prescription to be delivered to St Madoes, Glencarse Post Office or Inchture Shop but please allow an extra 24 hours for this. If preferred a paper script can be collected from the surgery

Disabled Persons

Our premises have suitable access for disabled persons.

Problems/Concerns

The practice operates a formal complaints procedure and a copy of which is available from the practice manager. If you have a complaint then we would ask that you contact the Practice Manager or Dr Easton in the first instance who will be happy to discuss the matter with you and if necessary a full investigation into the complaint will be carried out. We will keep you informed of the outcome of our complaint.

Patients Rights and Responsibilities

We provide medical care, so if you need to be seen urgently we will try to meet you request .Urgent appointments however can be shorter in length and you may have to wait to be seen. We will treat you as an individual and respect your privacy and dignity. In return we ask you to be polite to all our staff. We will, if necessary, ask you to join another practice if you use abusive language or physical violence towards our staff or other patients.

Please inform the staff if you have a change in circumstances i.e. surname, address and telephone number (if you are x directory)

Access to your Records

The practice is registered with Data Protection Register and your rights are protected by the Data Protection Act (1998). All our patient records are kept on computer, which are completely confidential. The staff directly involved with your care such as Doctors and nurses would normally see the information in your records. Our clerical staff will have limited access to your records to perform certain duties such as filing reports and case notes.

Information Sharing

The practice complies with both Data Protection Act (1984) & (1998) and access to Health Care Record Act (1990).

Identifiable information about you will be shared with others healthcare professionals if you require further medical treatment such as District Nursing & hospital.

Access to other services e.g. insurance companies, solicitors (will require your written consent) you have the right to see and alter any report before it is passed onto any second party.

Freedom of Information Act 2002 (Scotland). Any requests for information regarding this, we will endeavor to deal with your request within 20 working days. NHS Tayside may charge for handling a request.

We have a legal duty to inform others, e.g. child protection cases.

Patient information e.g. diabetic care is shared at a local & national level to help The Health Board & government plan services. If you do not wish information about you to be used in such a way please inform us.
